How to Get Your Crutches

Download your reference sheet

One page. It tells your provider exactly what to prescribe β€” bring it to your appointment.

Ask your VA provider

At your VA or Community Care visit, ask for in-Motion crutches by Millennial Medical. They add the code β€” E0110 for forearm or E0117 for underarm β€” to your prosthetics order.

Submit your order to Millennial Medical

Have your VA or Community Care provider submit and pay for your order directly to Millennial Medical β€” they can email it to orders@millennialmedical.com or fax it to 435-750-8989. We'll ship your crutches to your door, free of charge.

Find Your VA

Your Nearest VA Prosthetics Office

Your crutch order is handled by Prosthetics (PSAS) at your closest VA medical center. Enter your ZIP to find it.
