Tell your employer in writing and get your claim number and your claims adjuster's contact info. Everything — including your crutches — gets billed to the workers' comp carrier under that claim.

Ask your treating physician for forearm crutches

You have more say than you think. In many states you can choose your treating physician, and your prescription is where you request a specific model. Use this line:

"I'd like forearm crutches for my recovery — code E0110 — and please note the work-related diagnosis in my chart."
